Few prospective studies have investigated the risk factors for throwing-related shoulder and elbow injuries in the preseason. Shoulder and elbow injuries are common in baseball pitchers. Daily posterior shoulder stretching may reduce the incidence of the injuries in high school baseball pitchers. The incidence of injury was significantly lower in the S-group than in the N-group ( p = 0.04). HRs were 0.36 and 0.47 for the S- and SM-group, respectively, based on the N-group. The injuries occurred in 25, 35, and 57% of participants and median times to injury were 89, 92, and 29.5 days in the S- (n = 32), SM- (n = 46), and N- (n = 14) group, respectively. Log-rank test was used for between-group comparisons of survival distributions. Kaplan-Meier survival curves were generated and hazard ratios (HRs) for injury occurrence were calculated using multivariate Cox regression. Injury was defined as inability to play for ≥8 days because of shoulder/elbow symptoms. The pitchers freely chose to participate in one of four groups. Ninety-two pitchers participated in this study and were taught to perform stretching and strength exercises aimed at improving shoulder external rotation strength in the preseason. Thus, the throwing motion requires higher intensity eccentric contraction of the teres minor than the infraspinatus, which could lead to the teres minor being more fatigued and thus more muscle stiffness after pitching.We prospectively evaluated the effects of a prevention program on the incidence of shoulder and elbow injuries in high school baseball pitchers. Furthermore, at 90 deg of abduction (ie the position of the shoulder during throwing), the teres minor to infraspinatus muscle activity ratio is significantly higher than a 0 deg of abduction. Why is this relevant? For baseball pitches, the teres minor demonstrates the highest level of EMG activity of all the shoulder muscles during the deceleration phase. ➡️Whereas the sidelying horizontal abduction stretch decreased teres minor stiffness. ➡️Interestingly enough, the study found that the sleeper stretch decreased infraspinatus muscle stiffness. Both of these muscles make up what is known as the "posterior cuff." It is the posterior cuff, in addition to the posterior shoulder capsule, which contributes to posterior shoulder tightness. The cool thing about this study though, is that they looked at muscle stiffness of both the infraspinatus and teres minor using ultrasound. ‼️However a newer study by Yamauchi et al 2016 found that BOTH stretches effectively increased shoulder internal rotation and horizontal abduction with no significant differences between groups.‼️. A previous study had shown that the horizontal abduction stretch was more beneficial in restoring internal rotation at the shoulder than the sleeper stretch. Two other alternatives are shown here in the video: the sidelying sleeper stretch and the sidelying horizontal abduction stretch. There are many ways to stretch the posterior shoulder, one of which we highlighted a few weeks ago.
